If a serious incident happens in a charity, the trustees must deal with it and report it to the proper authorities, including the Commission. You can report a serious incident using the online Concern about a charity form.
Trustees should report all serious incidents - whether confirmed or only alleged - and explain what happened and how they are managing the situation.
